Our Approach
Housing Stabilization Services is a Minnesota Medical Assistance benefit. We assist clients with disabilities, including mental illness and substance use disorder, and seniors with finding and keeping housing.
Who is Eligible?
A member is eligible for our services if they meet the following criteria:
On Medical Assistance (MA)
18 years old or older
Have a documented disability
Experiencing housing instability
At risk of institutionalization
Our Services
Consulting
We’ll develop a person-centered plan that helps both you and us find out what you’re looking for in a home.
Transition
We’ll assist you with the housing search and application process from A-Z. You may even be eligible for assistance with moving expenses.
Sustaining
We want to make sure you’re able to keep your housing by supporting you with tasks that my be a barrier.
Accessibility
We accept clients in the Twin Cities and surrounding suburbs. Have trouble commuting for appointments? No worries – we’ve got you covered with remote assistance.
